Adults who are applying for an Australian passport should use the Overseas Passport Application Form if:
. their previous Australian passport expired before 1 January 2006.
. their previous passport was issued with less than two years validity (for example, a Limited
Validity Passport or Emergency Passport)
. their previous passport was issued to them when they were 15 years old or younger
the customer cannot present the previous passport, or it was reported Lost or Stolen and
Voided, or it is so seriously damaged it can no longer be used to identify the holder
the customer wishes to update any of their personal details (apart from the name or spelling of their Place of Birth due to a recognised geographical renaming).

If you’ve taken your spouse or partner’s family name, or added it to your own family name, you can generally link your names by showing us:
• a marriage, registered relationship or name change certificate issued by a Registry of Births, Deaths and Marriages in Australia AND
• your Australian birth certificate or Australian citizenship certificate
We don’t require a separate name change certificate if your name change is registered in an amended Australian birth certificate or Australian citizenship certificate.
We’ll accept a foreign marriage, registered relationship or name change certificate only if you can’t get an Australian equivalent because you were born overseas and you live overseas and you married or entered into the relationship overseas. You must have entered the marriage or relationship after you became an Australian citizen, and the certificate has to be legalised.

A lost/stolen passport can be replaced by a full validity passport. Fill out your application form (Australian Passport Overseas Application). Please do not use whiteout on the declaration section. If you make a mistake, you can cross it out and initial next to the change. If you have signed incorrectly, please fill in a new form. Online: https://online.passports.gov.au/. This link will allow you to confirm that you are eligible to apply for an Australian passport. Note: You cannot download blank forms. A statutory declaration is a solemn declaration of fact or belief, and may generally be made in relation to any matter. An affidavit is used to give evidence in court proceedings, while a statutory declaration is used to give evidence in most other circumstances.
You are responsible for the content of the Statutory Declaration. If you are unsure of what you are required to write on the Statutory Declaration, you should contact the Authority who has requested that you sign one. The Statutory Declaration must be signed in front of the Consular or Diplomatic Officer and cannot be witnessed by mail. It is not necessary to make an appointment and blank Statutory Declaration forms may be obtained at the Australian High Commission and Australian Consulate-General. You will be required to present original current photo ID, showing both your current name and signature. You are responsible for forwarding the Statutory Declaration onto the relevant Authority. ***Fees apply***

Consular Officers can certify true copies of some documents and photographs. You are required to provide the original copy of the document. We cannot accept photocopies. There's a fee for this service. Making a certified copy of a document (per page of certified document) each document (Passport, Driver's Licence, etc) Must be on it's own page. From 1 Feb 2012 a CNI can be issued if, on the basis of info supplied by the applicant, the couple meet the criteria which would render a marriage legal in Australia:
• the parties are not closely related
• they are both of marriageable age (18 yrs)
• neither is already married.

With the exception of the marriage of same-sex couples, if there is any doubt about whether the marriage will be recognised in Australia, the certificate should not be issued, and the matter should be referred to Consular Policy and Training Section of DFAT which will consult the Attorney-General's Department as necessary.
Parties to a marriage should consult a solicitor if unsure that their marriage will be recognised in Australia, including a doubt that an overseas divorce is recognised by Australian authorities.
